Cam bearing shot after break in

Ok first for me on a new set of cam bearings wiped out after break in. This is a stock rebuild and awesome oil pressure. Ran about 15 minutes after a hot valve adjustment then started missing after a stuck exhaust valve due to oil loss to top end . The bearings were dura bond and look like the Babbitt just came loose. Really weird with stock valve springs.

Whenever I see the cam bearing material delaminating, oil starvation at the cam journal does come to mind.
 
How many cam bearings failed? Did the camshaft turn freely when installed?  Oil holes properly aligned in their respective holes?  What rpm was the engine run at during the initial break in period?  Was the engine pre-lubed before starting?
